• Our Supplemental application completed.
• Acord 125 & 126-Commercial General Liability application (Only if GL coverage is requested)
• Five years of current loss runs for those lines of business that coverage is being requested.
• Narrative on loss in excess of $10,000.
• Two years of financials including balance sheet and income statement.
• Promotional Materials (i.e. brochures, web-site) used in describing services and operations offered by the applicant.
• Statement of Qualifications, including resumes of key staff members and professional licenses/certifications.
• Specimen contracts used with clients and subcontractors and copies of guarantees, warranties & hold harmless agreements furnished by the Named Insured.
